Membership Forms

Volunteer Coach Membership Form   [doc]  [pdf]   Updated for 2010-2011 seasonal year, previous year forms should not be used

Non-Volunteer Membership Form This form can only be completed through the NJYS Office. District Commissioners cannot issue a Coaches pass to a Non-Volunteer.  Packages for registering Non-Volunteers can be obtained from the NJYS office.  The completed package must be returned to the office for processing which may require a week to complete.  Non-volunteers cannot have their paperwork completed while they wait.
